	Winston-Salem, Forsyth Co., North Carolina, USA This unusual form of quartz occurs as a filling in voids left between other mineral crystals and is known locally as BOX & PLATE quartz. It is found in angular blocky pieces that often mimic a crystal form. This one is 4x5 inches and is in the Charles Creekmur collection.
