Improving Mindat.orgProblems uploading photos

I've been attempting to upload photos and have encountered a problem. I was able to upload a parent photo and one child photo. When I attempted to upload a second child photo, the form for uploading the photo didn't load completely. Non of the blanks came up, so I was unable to select a photo or add/edit the details. Has anyone else encountered this problem?

Some additional information may be useful:
When clicking the button to add another photo of a specimen, I'm taken to the page where I am to select the image for the child photo and enter the data. However, nothing appears after the line "Select a File". It won't even allow me to choose a file. Is there now a limit on how many child photos one can add? Any help you can provide will be greatly appreciated. Thanks in advance.
